We will continue to use your Medicare card and process that for you but you will also need to pay a co- payment or ‘gap’ as detailed below.
· $15 - $50 for consultations with Dr Zonaga and Dr Lam
· $15 - $125 for consultations with Dr Richardson and Dr Yao
Private Patients
Initial consults $214.75 ($89.75 rebate from Medicare).
Subsequent consultations cost between $72.10 and $189.75 (between $32.10 and $89.75 rebate from Medicare) depending on length and type of consultation.
For consultations that attract a Medicare rebate we will put in the claim to Medicare for you. All you have to pay is the gap amount. (The difference between the rebate and the fee). Payment is due at the time of consultation and EFTPOS facilities are available. At present the Medicare cheque is sent to you but is payable to the Doctor. Please bring or post this to the Surgery as soon as possible.
Items that do not attract a Medicare Rebate include: Insurance reports, Solicitor’s reports, Workers Compensation, Third Party, Medicals for driving, diving and work.

Nobody likes to be kept waiting and we try hard to keep on schedule but medical emergencies can not be foreseen. One way to alleviate waiting times is to ensure that you book an appropriate length appointment. The receptionist can assist you if you are unsure how long you need.
Standard appointments are 15 minutes and only enough time to discuss one problem.
Long consultations are available on request and are necessary for the following:
Complex medical problems, PAP smears, Medicals, Diabetes and Asthma checks, Results, Multiple problems, new patients with complex history, Hormone Replacement, Counseling, Workers Compensation, and Procedures.
Repeat Scripts
There is a fee of $5.00 for scripts issued without a consultation. These normally take three working days. In some cases repeats will not be issued without a consultation as regular checks are advisable when taking certain medications.
Referrals
Re- referrals can be issued without a consultation and the fee is $5.00. These normally take three working days. We regret that it is illegal to backdate referrals, so please ensure you have a current referral before you visit a specialist. Referrals cannot be back dated. New referrals can not be issued without a consultation.
Phone Calls
You can contact the Practice Sister by phone during normal surgery hours. If she is unable to help a message will be taken and the call returned as soon as possible.
Research
From time to time we participate in research programs, your written permission will be obtained before any information, which is usually de–identified, is used.
Feedback
Please talk to the Doctor, Practice Manager or receptionist if you are experiencing any problems with the service we are providing.
We believe that problems are best handled in the practice but should you wish to you can contact the NSW Health Dept at Strawberry Hills NSW.
